The Case Management Curriculum provides a structured, strengths-based approach to helping birth parents identify needs, access services, overcome barriers, and navigate support systems across the lifespan. The curriculum guides case managers through assessment, service planning, goal development, resource linkage, advocacy, follow-up, crisis response, and transition planning.

Birth parents receive support in areas such as mental health, healthcare, education, employment, housing, financial assistance, parenting, legal needs, and community connection. The curriculum incorporates engagement strategies based on the awareness-to-engagement continuum, ensuring low-barrier access and ongoing relationship building while promoting participant choice, empowerment, cultural responsiveness, and continuity of care.
